Chronic Illness Can Make Life Hard, But THIPA’s Care Management Team Can Help

THIPA takes care of thousands of members who face chronic health conditions. That’s why we’ve designed our Care Management department to help make it easier for our members to heal at home safely and comfortably.

Whether it’s providing help to organize medications, scheduling doctors’ visits, even arranging for transportation or in home help, THIPA’s Care Management team is here to help you coordinate your care so you don’t have to do it all alone.

“If you’re over 65 and recently visited the Emergency Room, you’ll receive a phone call from us to talk about our services,” says Branka Jerkov, R.N., Care Management. “We help coordinate your medical care by assisting with your discharge plan of care and communicating with your primary care physician. Our goal is to connect you with any resource that you may need to heal more quickly.”

You can also call the Care Management Department any time you wish. Dial 310-257-7250 ext. 55343. You’ll speak with a case manager who will conduct a brief, confidential pre-screening to determine if Care Management services would be beneficial to you. There is no obligation to accept.

82-year-old Denise deals with joint pain and breathing difficulties every day. Her son cares for her but soon his job will make providing assistance difficult. Denise needed help. Thankfully, THIPA’s Care Management team was there.

Care Management arranged for her to see a specialist for her pain. They found another caretaker to help Denise while her son worked. They also connected her to a utility program to provide financial relief.

“I am grateful that THIPA was able to quickly get me an appointment with the specialist I needed, in addition to helping me find the right in home care and transportation services,” says Denise. “As a result, I can now safely and comfortably stay in my home while my son is at work.”

When the Care Management team followed up with Denise they learned that the specialist has helped dramatically reduce her pain and improve her breathing. Not only that but the utility program Care Management connected her with reduced her utility bill by 20%!
