Extra Help to Heal at Home Safely and Comfortably

You may be one of the millions of older adults who face one or more chronic health conditions. You’re taking handfuls of daily medications, juggling visits to doctors and specialists and even stays in the hospital. It’s a lot to handle, even on your good days.

That’s why we’ve designed our Care Management programs to help make it easier for THIPA members to heal safely and comfortably at home.

“We can help you organize your medications, assist in coordinating doctor’s visits, and even arrange for transportation,” says Carol Ecklund, THIPA’s Director of Medical Management.

“We understand how difficult it is to manage life at home when you’re struggling with chronic illness.”

Carol, a nurse who has worked in oncology, hematology, medical and surgical areas for 20 years, leads a team of nurses and social workers who work very closely with THIPA members and their caregivers.

“If you’re over 65 and recently visited the Emergency Room, you’ll receive a phone call from us to talk about our services,” says Branka Jerkov, R.N., Manager, Ambulatory Care Mangement. “We help you with your discharge plan of care and communicate with your primary care physician. Our goal is to connect you with any resource that you may need to heal more quickly.”

THIPA Care Management services are free to all THIPA members. You can self-refer to care management and a physician order is not needed.

As a result, THIPA members are getting the best health care possible and I’m proud of that.”
